What is going on?

The Examine phase of the SSA process addresses the following question about the system under examination: 'What is going on?'. It is important to do this first because it is necessary to understand what is happening in the system before it can be effectively determined which parts of it are likely to go wrong. This is key to doing a rigorous assessment.
